Here is the link for the 2010-2011 IHSAA Physical Form. Starting May 1, 2010, physicals for the 2010-2011 school year can obtained from your local physician. Any physical prior to this date, will not be accepted for the 2010-2011 school year. Copy and paste the following link into your address bar:
